Required Paperwork
On the day of tryouts, please bring a completed Medical Release form.
Athletes will also need to bring in a copy of their USAV membership card. Please see the link below to purchase your USAV membership. You will be directed to the AZ Region Website. Please read and follow all directions to obtain a membership.
All paperwork will be submitted after team selection the day of tryouts. Coaches will collect all required paperwork from the athletes that have been selected.
